Supportive Housing

CPC offers Supportive Housing throughout Monmouth County. The scope and purpose of the program is to increase independence, community integration and quality of life.

Services include:

  • Completion of comprehensive rehabilitation needs assessment for consumer-directed treatment

  • Development of individualized plan for recovery and wellness

  • Skills development training and skill teaching based on:

    • Daily living skills

    • Social skills

    • Navigating the physical and mental health systems

    • Budgeting, financial stability and entitlements

    • Recreation and leisure

    • Self-advocacy

    • Wellness and a healthy lifestyle

    • Employment, volunteer and educational readiness

  • Coordination of care with individual's family or support system

  • In-community care management

  • Crisis Intervention

  • Supportive counseling

  • Family psychoeducation and support
