Adult Partial Care provides a full day of treatment, wellness and psychiatric rehabilitation services for people with psychiatric and/or co-occurring disorders.
The treatment team, along with the client and his/her support network, collaborate on an individualized recovery plan that includes goals and addresses the steps necessary to achieve them. Those steps incorporate participation in a variety of treatment groups along with medication management and case management services. There is an emphasis wellness and physical healthcare in addition to assisting with a plan to increase socialization and decrease isolation. Family is an essential component of this process.
Consumers may be involved in program daily, or less often, depending on individualized recovery plans and goals. Treatment teams consist of adult psychiatrists, licensed clinical staff and case managers. Psychiatric assessment and medication management are a component of partial care services.
Supported employment services, and assistance with benefits and housing, are provided based on each consumer’s individualized recovery plan goals.
Transportation services are either arranged for the consumer or are provided by CPC.
The partial care program hours are: 9:00 a.m. – 3:15 p.m., Monday through Friday at the Agency’s Aberdeen Counseling Center location. There is an evening program for clients who cannot attend during the day. To make an appointment call 732 290-1700 x5116.
