Behavioral Health Home

The Behavioral Health Home (BHH) offers a whole-person approach to the integration of physical, mental health and substance abuse treatment through case management, health promotion and support services.  The BHH office is located at the Aberdeen Counseling Center (ACC) and is available to adults 18 and older who experience serious mental health or co-occurring disorders as well as chronic medical conditions and are enrolled in Medicaid.

The Behavioral Health Home offers support and linkage to primary care providers, specialists, as well as offering lab and diagnostic procedures to meet the client’s healthcare needs and to provide continuity of care. CPC’s experienced staff will assess, plan and coordinate treatment for health issues utilizing a holistic, person centered and respectful approach. BHH clients can also receive psychiatric services in one of CPC’s treatment locations, if necessary to meet the client’s needs.

Upon admission into the Behavioral Health Home, clients receive a thorough assessment of acute and chronic health problems by an experienced Registered Nurse who has experience working with clients who experience mental health and addiction issues in addition to their physical health conditions.  Our dedicated team of professionals will schedule primary care provider appointments or work with the primary care provider the client is already engaged with, and arrange diagnostic studies and lab work as needed.  Transportation to appointments is available, and the Behavioral Health Home staff will act as liaison between the client and primary care provider in order to achieve the best outcomes for each client.

The Behavioral Health Home includes an Advanced Practice Nurse (APN), a Registered Nurse, a social work care manager, and a peer specialist.  This staff will provide as much oversight and follow up as necessary to ensure that clients receive the treatment they need to live the healthiest life possible in the community.  These services include scheduling appointments, assisting with medication, providing education about health concerns and treatment modalities, assisting with diet and nutritional planning, exercise, as examples.   CPC Behavioral Health Home provides continual education related to health maintenance, healthy living lifestyles and tips and strategies in order to maintain optimal health.

The Behavioral Health Home is conveniently located in the CPC Aberdeen Counseling Center, 1088 State Route 34, Aberdeen, New Jersey.

Hours of Operation are 8:30 a.m. – 4:30 p.m., on Mondays, Wednesdays, Thursdays and Fridays, with extended hours on Tuesdays from 8:30 a.m. until 7:00 p.m.  Please call: (732) 290-1700.
